This article explains to create surveys using Zoho Survey.

As soon as you access the application, to the center you will see all existent surveys [1]. To the left, you have access to create a new survey and a summary of your surveys [2]:
To edit, clone, preview, publish or delete a survey, you can use the options located to the right of it's name:

Create a survey
To create a new survey, click on the "Create survey" button to the left:
Enter the name and category and clic on "Create":
Now, you will have an empty canvas you can use to add elements. There are a lot of possible types of questions to be used when building a survey. You can check them to the left of the page:
Double click or drag and drop a question type into the canvas:
Enter all required information like question, answers, if it's mandatory, choices and so on:
Remember: you can change the format of the text, insert images, links and other elements using the controls:
You can use the "Advanced options" to change things like the number of columns, change the order of choices, mark choices as exclusive, prepopulate answers, among others:

Display logic and answer display logic
The "Display logic" and "Answer display logic" menus help you configure advanced logic for the question. Let's say for example that you want you have a "How was the service?" question. If the customer answers "bad" you would like to enable another question requesting for more information. This could be done by using the "Display logic" option. To do this, we will create a new question and configure it's values accordingly in the "Display logic" tab:
In this example, the second question is going to be displayed when "How was the service?" is answered as "Bad". You can add options to hide a question when certain criteria is met or to always / never show.
The "Answer display logic" allows you to define when a questions answer is going to be shown. Let's say you want to have a "Would you recommend us?" question but, if the customer rated the service as "Bad" then you only want it to display two possible answers: "No" or "Not sure". To achieve this, in the "Answer display logic" tab specify the "Display these choices only if a certain condition is met" [1] and select the two desire choices [2]. Then, in the condition [3], select that this will happen when the "How was the service?" question is answered with a "Bad" value:
Click "Save" when the question is completed. Add all the questions needed. Zoho Survey offers a lot of types to complete your form.
You can click on the "Untitled" area to change the name of the page:
